Are you taking Accutane (or similar) or have you in the past year?*
Brand names include: Accutane, Claravis, Sotret, Accutane, Amnesteem, Myorisan
Using retinoids (Accutane or similar) can cause skin to rip off during the waxing process. For your own safety, we cannot wax you until you have been off of Accutane or similar medicatiosn for over a year's time.
I understand the dangers of Accutane. Should I decide to take Accutane in the future, I will inform my Sugarista and will not get sugared at Sugaring LA at any time within one year of taking the medicine.
This section contains the Liability Waiver and release for Sugaring LA, along with further representations and warranties. By typing your name into the box at the end of this form, you agree to waive and release all rights to any and all claims, demands, causes of action, liabilities, and damages (collectively “Claims”) against Sugaring LA associated with the sugaring and skin care services performed at your request, unless such Claims arise from the gross negligence or willful misconduct of Sugaring LA or its Sugarists. By typing your name into the box at the end of this form, you further represent and agree to the following statements in the remainder of this section:
I have completed this form to the best of my ability. I will consult with my Sugarist regarding any medicine I am currently taking and any skin tendencies that may be problematic. I give permission to my Sugarist to perform the sugaring treatment or skin care procedure that I have requested. I have given an accurate account of the questions asked above including all known allergies and all prescription drugs or products I am currently ingesting or using topically. I understand my Sugarist will take reasonable precautions to minimize or eliminate negative reactions as much as possible.
I have read and understand post-treatment home care instructions. I am willing to follow recommendations made by my Sugarist for a home care regimen that can minimize or eliminate possible negative reactions. In the event that I may have additional questions or concerns regarding my treatment or suggested home product / post-treatment care, I will consult with the Sugarist immediately.
I agree this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures. I certify that I have read, and fully understand the Liability Waiver and that I have had sufficient opportunity for discussion to have any questions answered. I understand the procedure and accept the risks.
LATE CANCELLATION + NO-SHOW POLICY
We understand, life can be unpredictable! You may cancel your appointment BY 8AM the day of your visit, with no penalty. However after 8AM, Late Cancellations (and No Shows) will result in a fee of 50% of the total booked service(s) that will be charged on your next visit.

Your appointment time is reserved exclusively for you, so please understand that "no-shows" and late cancellations are an inconvenience to both our Sugarists and turned-away clients. When late cancellations and no-shows happens, our Sugarists miss out on the possibility of filling the appointment time, and other clients lose the opportunity to receive services.
If you do not show up for your appointment within 5 minutes of your reserved appointment time, we will need to consider that a "no-show"...unless we can fit you in before the next appointment or with a different Sugarist.
If you schedule your appointment within 24 hours of your appointment time, a cancellation fee of 50% of the total booked service(s) will apply if you do not give us notice of at least three (3) hours before your appointment time.
Also as courtesy to our clients, we typically try to send both text and email confirmations about 48 hours prior to your appointment. However, it is your sole responsibility to remember your appointment time and to adhere to Sugaring LA’s cancellation policy.
This policy is enforced as a courtesy to let other clients know our availability, as well as to respect the time of our Sugaring LA Sugarists. All Sugaring LA policies are displayed in our salons, on our website, and in your email confirmation. Sugaring LA reserves the right to change, add, remove, or otherwise modify our policies at any time without prior notice.
INFORMATION WE COLLECT
Sugaring LA has a variety of ways in which we may obtain information about you. All information gathered is offered directly from the client and you may provide it in one of our salons, on our website, at events, or on the Mindbody app. When you visit us online on our website, access features in our Mindbody app, or stop by our social media pages, we may collect certain automated information about your device, technologies, cookies, internet protocol (IP address), and server logs. This information helps to provide a more personalized experience for you both in the salon and online.
PROVIDED INFORMATION AND CONSENT
We request certain personal information to be provided by the client prior to any appointment for service(s) and the types of information you provide to us may include basic contact details (name, address, email address, mobile phone number), your age and date of birth, gender, skincare characteristics and concerns, and payment information - including card number, expiration date, security code, and billing address. When you provide us with these personal details, such as when you book an appointment for service(s) or verify your credit card information, it is inferred that you consent to us collecting this information and using it only for that specified purpose. We collect and store this information in order to book and hold all appointments for service(s) and to provide stellar customer service.

California Civil Code Section § 1798.83 permits users of our website and our Mindbody app that are California residents to request certain information regarding our disclosure of personal information to third parties for their direct marketing purposes, which we do not do. CHILDREN UNDER THE AGE OF 13
Our website and services are not intended for children under 13 years of age. No one under age 13 may provide any information to the website. We do not knowingly collect personal information from children under 13. If you are under 13, do not use or provide any information on our website, or on or through any of its features, and do not register on the website or in our Mindbody app, or provide any information about yourself to us, including your name, address, telephone number, e-mail address. If we learn we have collected or received personal information from a child under 13 without verification of parental consent, we will delete that information. If you believe we might have any information from or about a child under 13, please contact us at firstname.lastname@example.org.
We have implemented measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration, and disclosure. Unfortunately, the transmission of information via the internet is not completely secure. Although we do our best to protect your personal information, we cannot guarantee the security of your personal information transmitted to our website or in the Mindbody app. Any transmission of personal information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained on the website or in the Mindbody app.
